The occupational-disease issue made more sense once I separated gradual exposure from direct trauma, because the proof may build across medical and work records. A later change in ability to work can become a reopening fact when it is tied to updated restrictions. Whether someone uses a lawyer is different from why the review request was denied. That left me with a simple sequence: label first, reason next, changed fact after that, procedure at the end.
